When to Use

Screening for generalized joint hypermobility (GJH) in athletes presenting with recurrent joint subluxations, dislocations, or soft tissue injuries.

Pre-participation evaluation for sports with high flexibility demands (e.g., gymnastics, dance, swimming) to identify athletes at risk for joint instability.

Assessment of patients with suspected hypermobility spectrum disorder (HSD) or Ehlers-Danlos syndrome (EDS), particularly the hypermobile type (hEDS).

Evaluation of chronic musculoskeletal pain, fatigue, or dysautonomia symptoms in young, active individuals.

Differentiating GJH from other causes of joint pain or instability in sports medicine clinics.

Key Indication - Diagnostic Criteria for Hypermobility Spectrum Disorders

The Brighton Criteria are primarily used to diagnose generalized joint hypermobility and classify hypermobility spectrum disorders. A score of ≥4/9 on the Beighton scale (with or without additional major/minor criteria) indicates GJH. The criteria are endorsed by the International Consortium on Ehlers-Danlos Syndromes for identifying hEDS when combined with other features. In sports medicine, a positive Brighton score warrants further evaluation for joint instability and tailored training modifications.

When Not to Use / Contraindications

Acute joint injury or inflammation: Do not perform passive range-of-motion tests in acutely injured joints (e.g., recent dislocation, fracture, or severe sprain).

Post-surgical joints: Avoid testing in joints that have undergone recent surgery (e.g., ligament reconstruction) as results may be confounded.

Children under 5 years: The Beighton scale is less reliable in very young children due to physiological hypermobility.

Patients with known connective tissue disorders other than hEDS (e.g., classical EDS, vascular EDS) where specific diagnostic criteria apply.

When the patient is unable to cooperate or has cognitive impairment affecting reliable self-report of symptoms.
