When to Use

Pre-participation screening for female athletes at risk of the Female Athlete Triad (low energy availability, menstrual dysfunction, low bone mineral density).

Annual surveillance in collegiate, elite, or adolescent female athletes participating in endurance, aesthetic, or weight-class sports (e.g., distance running, gymnastics, figure skating, rowing, lightweight rowing, martial arts).

Clinical evaluation of athletes presenting with stress fractures, recurrent injuries, disordered eating, or amenorrhea/oligomenorrhea.

Monitoring of athletes undergoing return-to-play protocols after a Triad-related injury or diagnosis.

Screening tool in sports medicine clinics, athletic training rooms, and team physician offices to identify athletes requiring further metabolic, endocrine, or bone health assessment.

Key Indication - Identifying Low Energy Availability Risk

The LEAFQ (Low Energy Availability in Females Questionnaire) is a validated screening tool designed to identify female athletes at risk for low energy availability (LEA), a key component of the Female Athlete Triad and Relative Energy Deficiency in Sport (RED-S). A score ≥8 indicates elevated risk, warranting further clinical evaluation by a sports medicine physician, registered dietitian, and/or endocrinologist. The questionnaire covers three domains: physiological symptoms, menstrual function, and history of injuries/bone health. It is recommended by the International Olympic Committee (IOC) and the American College of Sports Medicine (ACSM) as part of routine screening in female athletes.

When Not to Use / Contraindications

Male athletes (the LEAFQ is validated only for females; use the LEAM-Q or other male-specific tools for male athletes).

Athletes currently using hormonal contraception (e.g., oral contraceptives, IUDs, implants) that alter menstrual bleeding patterns, as menstrual domain scores may be unreliable.

Postmenopausal women or those with primary amenorrhea due to anatomical or genetic causes (e.g., Turner syndrome, Müllerian agenesis).

Athletes with known eating disorders currently under treatment (the tool may still be used but results should be interpreted cautiously).

Acute illness or injury that may temporarily affect energy availability or menstrual function (e.g., recent surgery, infection).
