When to Use

Young to middle-aged active adults (typically 20–40 years) presenting with insidious onset of groin pain, hip stiffness, or mechanical symptoms such as clicking, catching, or locking.

Athletes involved in sports requiring repetitive hip flexion and rotation (e.g., soccer, hockey, ballet, martial arts, golf) with activity-related hip or groin pain.

Patients with a positive impingement test (flexion, adduction, internal rotation) on physical examination, suggesting femoroacetabular impingement.

Pre-participation screening in high-risk sports to identify asymptomatic individuals with cam or pincer morphology who may benefit from preventive strategies.

Surveillance of patients with known hip dysplasia or prior hip surgery for development of secondary impingement.

Differentiation of intra-articular hip pathology from extra-articular sources (e.g., iliopsoas tendinopathy, athletic pubalgia, adductor strains) when clinical presentation is ambiguous.

Key Indication - Diagnostic Criteria for FAI Syndrome

The Warwick Agreement (2016) defines FAI syndrome as a triad of: (1) symptoms (motion-related or position-related hip/groin pain, stiffness, clicking, or locking), (2) clinical signs (positive impingement test, restricted range of motion, especially internal rotation in flexion), and (3) imaging findings (cam or pincer morphology on radiographs, CT, or MRI). All three components must be present for diagnosis. This consensus standardizes diagnosis and guides management, emphasizing that imaging alone is insufficient without correlative symptoms and signs.

When Not to Use / Contraindications

Acute traumatic hip injury (e.g., fracture, dislocation) where urgent orthopedic evaluation is needed.

Septic arthritis or other infectious causes of hip pain (fever, erythema, elevated inflammatory markers).

Advanced hip osteoarthritis (Kellgren-Lawrence grade 3 or 4) where impingement is a secondary phenomenon and joint replacement may be indicated.

Avascular necrosis of the femoral head, which presents with similar symptoms but requires different management.

Referred pain from lumbar spine, sacroiliac joint, or intra-abdominal pathology (e.g., hernia, gynecologic conditions) – FAI syndrome is a diagnosis of exclusion for intra-articular hip pathology.

Patients under 18 years with open physes, as impingement morphology may be developmental and not yet fully established.

Asymptomatic individuals with incidental cam/pincer morphology on imaging – do not label as FAI syndrome without symptoms and signs.
