When to Use

Pre-participation screening for athletes in high-flexibility sports (gymnastics, dance, swimming, volleyball) to identify generalized joint hypermobility (GJH).

Evaluation of patients with suspected hypermobility spectrum disorders (HSD) or Ehlers-Danlos syndromes (EDS), particularly hypermobile EDS (hEDS).

Assessment of joint laxity in patients presenting with recurrent joint dislocations, subluxations, or chronic musculoskeletal pain without inflammatory cause.

Screening for GJH in patients with chronic fatigue, fibromyalgia, or dysautonomia, as hypermobility is a common comorbidity.

Baseline assessment in rehabilitation settings to guide exercise prescription and avoid overstretching in hypermobile individuals.

Research studies investigating the prevalence of GJH in specific populations or its association with injury risk.

Key Indication - Hypermobile Ehlers-Danlos Syndrome (hEDS) Screening

The Beighton Score is a core component of the 2017 International Diagnostic Criteria for hEDS. A score ≥5 (or ≥4 in adults >50 years) is required for the first criterion (generalized joint hypermobility). It is used alongside other clinical features (skin, soft tissue, chronic pain, family history) to establish the diagnosis. The score is also recommended by the American College of Medical Genetics and Genomics for initial hypermobility assessment.

When Not to Use / Contraindications

Acute joint injury or recent surgery: Do not perform active range-of-motion testing if it may exacerbate injury or disrupt healing.

Inability to cooperate: Unconscious, uncooperative, or cognitively impaired patients cannot reliably perform the maneuvers.

Localized joint pathology: Do not test a joint with known arthritis, fracture, or infection; the score is invalid for that joint.

Age <2 years: Not validated in very young children; joint laxity is normal in infancy.

Pregnancy: Hormonal changes increase laxity; scores may be falsely elevated.

Neuromuscular disorders: Conditions like cerebral palsy or muscular dystrophy alter joint mobility and confound results.
