When to Use

Assessment of acute mountain sickness (AMS) in individuals ascending to altitudes above 2500 meters (8200 feet).

Pre-participation screening for trekkers, climbers, skiers, and military personnel deploying to high altitude.

Surveillance of AMS symptoms during staged ascent expeditions or rapid ascents (e.g., helicopter drops, high-altitude training camps).

Clinical evaluation in emergency departments or mountain clinics for patients presenting with headache, nausea, dizziness, or fatigue after recent altitude exposure.

Research studies investigating AMS incidence, prevention, or treatment interventions.

Key Indication - Diagnosis of Acute Mountain Sickness

The Lake Louise AMS Score is the most widely used diagnostic tool for acute mountain sickness. A total score of ≥3 points, including at least 1 point from headache, indicates AMS. This threshold is endorsed by the Wilderness Medical Society and the International Society of Mountain Medicine. The score should be assessed after at least 4 hours at the current altitude to ensure symptoms are altitude-related and not due to other causes.

When Not to Use / Contraindications

Do not use in patients with high-altitude cerebral edema (HACE) or high-altitude pulmonary edema (HAPE) as the primary tool; these require immediate descent and specific management.

Not valid for children under 5 years of age; alternative pediatric scoring systems (e.g., Children's Lake Louise Score) should be used.

Avoid use in individuals with pre-existing neurological conditions (e.g., migraine, concussion) that may confound symptom reporting.

Not reliable in patients with acute illness (e.g., viral infection, dehydration) that mimics AMS symptoms.

Do not use as a sole criterion for diagnosis if the patient has taken prophylactic medications (e.g., acetazolamide) that may alter symptom perception.
