When to Use
The AAST Vascular Injury Scale is indicated in all patients with suspected major vascular trauma identified on CT angiography (CTA), conventional angiography, or during operative exploration. It is essential in penetrating extremity, neck, torso, and junctional injuries, as well as high-energy blunt trauma with associated fractures or dislocations. The scale helps stratify severity and informs limb salvage versus amputation decisions, particularly when combined with the Mangled Extremity Severity Score (MESS).
The scale is purely anatomic and does not replace physiologic assessment. Hemodynamic instability or hard signs of vascular injury mandate immediate operative exploration without delay for detailed imaging or grading.
