When to Use
The ABC Score is indicated for adult trauma patients upon arrival to the trauma bay to rapidly identify those at high risk of requiring massive transfusion (≥10 units packed red blood cells in 24 hours). It is particularly useful in blunt and penetrating trauma when the team is deciding whether to activate the Massive Transfusion Protocol (MTP).
The ABC Score should not delay resuscitation. It is a decision-support tool only and performs best when used in conjunction with clinical judgment and other markers such as base deficit and lactate.
