BWPS is sensitive but NOT specific. Sepsis in a patient with untreated hyperthyroidism can easily score >45 points without being a true primary thyroid storm. Treat both parallelly.
Always look for the trigger: infection (most common), surgery, trauma, withdrawal of anti-thyroid drugs, iodinated contrast load, or vigorous palpation of the gland.
Developed by Drs. Burch and Wartofsky in 1993, standardizing the largely subjective clinical impression of thyroid storm.
