The T-score compares the patient to a young-adult reference population. The Z-score compares the patient to an age, sex, and ethnicity-matched population. A Z-score ≤ -2.0 suggests an underlying "secondary" cause for bone loss (e.g., hyperparathyroidism, malabsorption, myeloma).
Presence of a vertebral or hip fracture is diagnostic of osteoporosis regardless of the BMD T-score. These patients should be treated.

The T-score compares BMD to young healthy adults. In premenopausal women and men < 50, use the Z-score (comparison to age-matched peers) instead.
