Modern ultrasound machines (high-frequency) reveal many more follicles. International consensus guidelines now suggest a threshold of ≥ 20 follicles per ovary when using transducers with ≥ 8 MHz frequency.
PCOS is a diagnosis of exclusion. Even if a patient meets 2/3 criteria, the diagnosis is invalid if a mimic like NCAH is not ruled out with a morning 17-hydroxyprogesterone level.
