In older patients or those with high body fat percentage, the TBW factors are typically lower (0.5 for men, 0.45 for women).
The free water deficit is only one part of the total fluid deficit in HHS. Most HHS patients have a total deficit of 8 to 12 Liters (approx. 10–15% of body weight), involving both free water and isotonic salt losses.
Hypernatremia.
Deficit = TBW × [(Na / 140) - 1]. Total Body Water (TBW) is approx. 60% of weight in men and 50% in women (lower in elderly). Note that total fluid deficit in HHS includes both free water and isotonic losses, often totaling 8–12 Liters.
