For fever ≥ 38°C, minor infections (UTI, URI), or minor dental procedures. Take twice your daily maintenance dose until the illness clears.
For fever > 39°C, significant trauma, or moderate surgical procedures. Take three times your daily maintenance dose, split into 3 or 4 administrations.
If you are vomiting, have severe diarrhea, or have major trauma, use your emergency hydrocortisone injection kit immediately (100mg IM) and seek emergency hospital care.
The half-life of oral hydrocortisone is short. During illness, it is better to take small doses frequently (every 6-8 hours) rather than one large double dose, to maintain more stable cortisol levels.
Usually, the dose of fludrocortisone (the mineralocorticoid part) does NOT need to be increased during sick days, as the high dose of hydrocortisone itself provides enough mineralocorticoid activity.
