While corrected calcium is helpful, Ionized Calcium (iCa) is the gold standard, especially in critically ill or acidotic patients, as it represents the physiologically active fraction.
Any calcium level > 14 mg/dL is a medical emergency regardless of symptoms. The primary risk is sudden cardiac death from QTc shortening and subsequent arrhythmias.
