When to Use

Primary prevention in European and high-income populations
Adults aged 40–69 without prior CVD, diabetes, CKD
Risk-based therapeutic decision-making (blood pressure, lipids)
ESC 2021 guideline–endorsed tool; preferred over SCORE1
Estimates 10-year fatal + non-fatal CVD events (unlike SCORE1)

Clinical Pearls

Key Distinctions from SCORE1

SCORE2 includes non-fatal events (≈2–3× multiplier vs. fatal-only)
SCORE1 underestimated true 10-year CVD burden by ~3×
Region-specific risk equations available (low, medium, high-risk regions)
Younger age (<40) requires risk modifiers; Framingham or other tools for <40 years

CV Risk Modifiers

Family history of premature CVD → escalate risk category
LDL calcification on CT → higher absolute risk
CKD (eGFR 30–59) → grade up risk category
Elevated lipoprotein(a) → consider additional statin/ezetimibe
LDL particle number / apoB preferentially used if available

Next Steps

Very Low (<1%) / Low (1–5%) Risk

Intensify lifestyle: exercise, Mediterranean diet, smoking cessation
Reassess CVD risk every 5–10 years or if new risk factors emerge
Pharmacotherapy not recommended for primary prevention

Moderate Risk (5–10%)

Assess additional risk modifiers (family Hx, calcium score, Lp(a))
If LDL remains >1.8 mmol/L despite lifestyle: initiate moderate-intensity statin
Blood pressure target <140/90 mmHg (lower if tolerated); consider RAAS inhibitor if HTN + albuminuria

High (>10%) Risk

Initiate statin ± ezetimibe; target LDL <1.4 mmol/L
Blood pressure target <130/80 mmHg if tolerated
Aspirin not routinely recommended in primary prevention (USPSTF 2022)
Annual reassessment; escalate to PCSK9i if LDL goals not met on statin + ezetimibe

Origins & History

Development

SCORE2 was developed by the European Society of Cardiology (ESC) SCORE project in 2021, updating the original SCORE system to include non-fatal events and modern European epidemiological data. Part of the ESC 2021 prevention guidelines.
