When to Use

Clinical Use

Staging of newly diagnosed HCC at the time of multidisciplinary tumor board.
Dictates the sequence of care (Surgery vs. Locoregional vs. Systemic).
Requires liver function (Child-Pugh/ALBI), tumor burden (imaging), and performance status (ECOG).

Clinical Pearls

Treatment Stage Migration

The BCLC algorithm is flexible. If a patient fails a stage-specific therapy (e.g., progression after TACE), they should "migrate" to the next stage's therapy (e.g., systemic therapy) even if their anatomical stage has not changed.

The Advanced (Stage C) Revolution

Since 2020, Sorafenib is no longer the first-line standard for Stage C. The Atezolizumab + Bevacizumab combination (IMbrave150) proved superior, provided the patient does not have untreated esophageal varices or high bleeding risk.
