Spinal stability is maintained. Mechanical intervention is typically not required; focus on systemic or radiation therapy.

When to Use

When to Use

Any patient with known spinal metastases or a suspicious vertebral lesion on imaging.
Used to decide if a patient needs an urgent surgical consultation for stabilization prior to, or in conjunction with, radiotherapy.
NOT used for primary bone tumors; specifically designed for metastatic involvement.

The 6 Scoring Variables

Posterolateral Involvement0-3 (Involvement of facets, pedicles, or laminae)

The "Mechanical Pain" Pearl

Pain that is relieved by lying flat and worsened by sitting or standing is "mechanical" and highly suggestive of instability. This is distinct from "biological" cancer pain which is often dull, constant, and worse at night.
