When to Use

Clinical Use

Interim PET assessment (iPET) after 2-4 cycles of chemotherapy to assess sensitivity.
End-of-treatment assessment (eotPET) to confirm complete metabolic response (CMR).
Used to decide on treatment escalation (e.g., ABVD to BEACOPP) or de-escalation.

How it Works

The 5-Point Scale

ScoreDefinition
1No uptake above background.
2Uptake ≤ mediastinum.
3Uptake > mediastinum but ≤ liver.
4Uptake moderately > liver.
5Uptake markedly > liver and/or new lesions.
XNew areas of uptake unlikely to be related to lymphoma (e.g., inflammation).

Clinical Pearls

Complete Metabolic Response (CMR)

A Deauville score of 1, 2, or 3 is generally considered to represent a Complete Metabolic Response in most modern trials. While Score 3 is higher than background, it is usually associated with a similarly excellent prognosis as Score 1-2.

The Liver Baseline

Score 4 and 5 represent "positive" scans. In the interim setting (iPET), a Score of 4 or 5 indicates a high risk of relapse and should prompt a discussion regarding biopsy confirmation or switching to a more intensive salvage regimen.
