Tyrer-Cuzick (IBIS v8)

IBIS V8Tyrer-Cuzick Risk Interpretation

Calculated Model Outputs

Clinical pearl

The 20% lifetime risk threshold is a key criterion for the American Cancer Society (ACS) recommendation for annual screening Breast MRI in addition to mammography. The 10-year risk is often used to discuss chemoprevention with SERMs or Aromatase Inhibitors.

Risk Profile

22% Lifetime

Management Path

High risk. Candidate for high-risk screening (e.g., annual MRI) and discussion regarding chemoprevention (NCCN/ACS guidelines).

When to Use

When to Use

Women without a prior personal history of invasive breast cancer or DCIS.
To estimate the 10-year and lifetime risk of developing breast cancer.
To identify high-risk individuals who qualify for enhanced screening (e.g., breast MRI) or chemoprevention (e.g., Tamoxifen, Anastrozole).

How it Works

The Integrated Risk Factors

Family History: Breast and ovarian cancer in first and second-degree relatives (including paternal side).
Reproductive History: Age at menarche, parity, age at first live birth, menopausal status.
Personal Factors: BMI, height, use of HRT.
Benign Breast Disease: Hyperplasia, atypical hyperplasia, LCIS.
Genetic Status: BRCA1/2 mutation status if known.

Mathematical Foundation

The model utilizes a Bayesian approach to calculate the likelihood of a woman being a carrier of a high-risk breast cancer gene based on her family tree, then adds the independent risk contributed by her hormonal and reproductive factors.

Actionable Thresholds

Risk LevelThresholdClinical Action
Average Risk< 15% LifetimeStandard screening (mammogram).
Moderate Risk15% - 19% LifetimeConsider enhanced surveillance.
High Risk≥ 20% LifetimeAnnual Breast MRI + Mammogram; discuss chemoprevention.

Tyrer-Cuzick vs. Gail Model

The Gail model is often criticized for its limited family history (only first-degree relatives). Tyrer-Cuzick is preferred for women with paternal family history or affected second-degree relatives, as it captures the underlying genetic risk more robustly.
