Staging Nuance

While the AJCC TNM 9th Edition provides detailed anatomic staging, the VALSG system (Limited vs Extensive) remains the primary driver of treatment decisions in clinical practice for SCLC.

When to Use

Clinical Use

All patients with biopsy-confirmed Small Cell Lung Cancer.
Initial staging to determine if the patient is a candidate for curative-intent concurrent chemoradiation.
Helps identify the extremely rare subset (Stage I-II, T1-2 N0) that may benefit from surgical resection.

Clinical Pearls

Immunotherapy Integration

The treatment of Extensive-Stage SCLC was revolutionized by the CASPIAN and IMpower133 trials. Adding a PD-L1 inhibitor to platinum/etoposide is now the global standard of care, providing the first overall survival benefit seen in this disease in decades.

PCI (Prophylactic Cranial Irradiation)

For Limited-Stage patients with a good response to therapy, PCI remains a standard consideration to reduce CNS relapse. In Extensive-Stage disease, MRI surveillance is increasingly used as an alternative to PCI in the immunotherapy era.
