ASTCT CONSENSUSCAR-T Toxicity Grading (CRS & ICANS)

Toxicity Severity

CRS Key Factors

Fever, Hypotension (BP), and Hypoxia (O2 Saturation).

ICANS Key Factors

ICE Score (orientation/writing), Level of consciousness, and Seizures.

When to Use

When to Use

Monitoring patients after infusion of CAR-T cells or bispecific T-cell engagers (BiTEs).
Used to determine the timing and dosage of Tocilizumab (IL-6 inhibitor) and corticosteroids.
Requires vital signs (BP, O2 sats) and the ICE (Immune Effector Cell-Associated Encephalopathy) score.

Clinical Pearls

The Fever Requirement

By definition, CRS Grade 1 requires a fever (≥ 38.0°C). Hypotension or hypoxia that occurs *without* a fever is not graded as CRS and should prompt investigation for other causes like sepsis or pulmonary embolism.

Management Trigger

Grade 2 CRS is the standard trigger for starting Tocilizumab. For ICANS, Tocilizumab is generally NOT effective (as IL-6 does not easily cross the blood-brain barrier); steroids are the primary therapeutic intervention for neurotoxicity.
