When to Use

Clinical Use

Baseline risk assessment of patients in Chronic Phase CML.
Estimating the probability of CML-related death.
Aiding the selection of first-line TKI (e.g., deciding between Imatinib or more potent 2nd generation TKIs like Dasatinib/Nilotinib).

How it Works

Scoring Variables

Age (Years)
Spleen size (cm below costal margin)
Blasts in peripheral blood (%)
Platelet count (x10⁹/L)

The Calculation

The ELTS score uses a complex exponential formula. Unlike Sokal, it assigns a higher weight to age and a lower weight to spleen size, reflecting the modern causes of death in CML patients.

Clinical Pearls

Superior Predictive Value

The ELTS score is more accurate than the Sokal score at identifying a truly "Low Risk" group that has almost zero probability of dying from CML. This makes it an excellent tool for identifying candidates for eventually attempting Treatment-Free Remission (TFR).

High Risk Implications

Patients in the "High Risk" ELTS band have a significantly higher risk of progression to accelerated phase or blast crisis. Many experts recommend starting these patients on a 2nd generation TKI (Dasatinib, Nilotinib, or Bosutinib) to achieve deeper, faster molecular responses.
