Scoring Definitions

  • Grade 1: Complete regression; thin linear fibrosis.
  • Grade 2: Dense fibrosis; no obvious residual tumor.
  • Grade 3: Fibrosis > visible tumor.
  • Grade 4: Visible tumor > fibrosis.
  • Grade 5: Massive tumor; no response.

When to Use

When to Use

Routinely used in the restaging MRI (typically 8-12 weeks post-completion of neoadjuvant radiation or Total Neoadjuvant Therapy) for mid-to-low rectal cancers.
Critical tool in multidisciplinary tumor boards to decide between Total Mesorectal Excision (TME) surgery versus a non-operative "Watch and Wait" protocol.

Clinical Pearls

The "Watch and Wait" Paradigm

Patients achieving an mrTRG 1 or 2, coupled with a normal digital rectal exam, negative direct endoscopy (no visible tumor, just a flat white scar), and normalized CEA, may be offered deferral of surgery. This "Watch and Wait" approach avoids the profound morbidity of permanent colostomy (APR) and severe genitourinary dysfunction.

Limitations and Pitfalls

MRI cannot detect microscopic residual disease. An mrTRG 1/2 is highly predictive of a pathological complete response (pCR), but ~20-30% of clinical complete responders will have a local regrowth within 2 years. Therefore, strict, frequent surveillance (q3 months with MRI and endoscopy) is mandatory.
