When to Use

When to Use

At the time of formal diagnosis of Active Multiple Myeloma.
Requires serum beta-2 microglobulin, albumin, LDH, and bone marrow FISH cytogenetics.
Used to define the risk of early relapse and overall survival, which dictates the necessity for aggressive triplet or quadruplet induction (e.g., Dara-VRd) and double-transplant or intensified maintenance strategies.

Clinical Pearls

The 1q Evolution

The most critical update in R2-ISS is the inclusion of the 1q chromosome abnormality. Previously ignored in standard R-ISS, 1q gain (3 copies) or amplification (≥4 copies) is now recognized as a massive driver of proteasome-inhibitor resistance and early relapse, earning its own points in the calculation.
