Clinical Criteria Checklist

Clinical pearl

The Z0011 trial demonstrated that for women meeting these specific criteria, addition of ALND did not improve 10-year overall survival or regional recurrence-free survival, but did significantly increase morbidity (lymphedema).

Patient does not meet ACOSOG Z0011 criteria. Axillary lymph node dissection (ALND) is traditionally indicated for positive sentinel nodes.

When to Use

Clinical Use

Applied to patients with early-stage breast cancer who are found to have 1 or 2 positive sentinel lymph nodes during surgery.
Used to decide if the patient can safely omit a completion ALND.
Specifically validated for patients undergoing breast-conserving surgery (lumpectomy) followed by whole-breast irradiation.

How it Works

Z0011 Inclusion Criteria

Clinical T1 or T2 tumor (< 5 cm)
Clinically node-negative (cN0) prior to surgery
1 or 2 positive sentinel lymph nodes (micrometastases or macrometastases)
Breast-conserving surgery planned
Whole-breast radiation planned
NO prior neoadjuvant chemotherapy

Clinical Pearls

Why ALND can be Omitted

The Z0011 trial showed that in patients meeting all criteria, the 10-year overall survival and regional recurrence rates were identical between those who had an ALND and those who did not. The tangential fields of whole-breast radiation likely provide enough coverage to "sterilize" any minimal residual disease in the remaining axillary nodes.

Contraindications to Omission

Gross extracapsular extension (ECE) in the sentinel nodes.
Mastectomy patients (though the AMAROS trial later expanded this concept to mastectomy if nodal radiation is used).
Patients receiving neoadjuvant chemotherapy (as the sentinel node may no longer represent the full axillary burden post-treatment).
