Lugano Classification

Disease Distribution

Note: Lugano staging integrates anatomic distribution with clinical symptoms (A/B) and extranodal extension (E).

Awaiting Anatomy

The Lugano classification is the clinical standard for staging Hodgkin and non-Hodgkin lymphomas.

Stage Tiers

  • Stage I: Single nodal region
  • Stage II: Multiple regions (same side)
  • Stage III: Nodal regions (both sides)
  • Stage IV: Widespread organs

When to Use

Clinical Use

Staging of both Hodgkin and Non-Hodgkin Lymphoma at diagnosis.
Assessing response to therapy (using the Deauville scale).
Determining the need for consolidation radiotherapy in bulky disease.

How it Works

Staging Framework

StageDefinition
ISingle nodal region or single extralymphatic site (IE).
IITwo or more nodal regions on the same side of the diaphragm.
IIINodal regions on both sides of the diaphragm, or nodes above with splenic involvement.
IVDiffuse or disseminated extralymphatic involvement (e.g., Liver, Bone Marrow, Lung).

Clinical Pearls

Response Assessment (PET-CT)

A Complete Metabolic Response (CMR) is defined by a Deauville score of 1, 2, or 3. The Lugano criteria simplified this by removing the need for a bone marrow biopsy for confirmation of CR if the PET-CT is negative, provided there was no suspicious bone marrow involvement initially.

Bulky Disease Definition

Lugano standardized the definition of "bulky disease" as a single nodal mass ≥ 10 cm in diameter, or ≥ 1/3 the transthoracic diameter in Hodgkin lymphoma. This is a critical factor in determining whether a patient receives chemotherapy alone or combined modality therapy.
